Registration Terms &
Conditions

Payment by Check:
Checks must be in US$ drawn on a US bank. Checks must be issued
within 2 weeks from the date of your invoice or arrive at the QFD Institute by October 10, 2008,
whichever comes first. (Please call 1-734-995-0847 ahead of time if you need to make other arrangements; a copy of a Purchase Order or Requisition Form may be
required.) Your registration will be confirmed upon receipt of full payment.

Payment by Wire Transfer:
This payment method is accepted for registrations totaling
US$1,500 or more, subject to availability of the bank service. Wiring instructions will be emailed to you upon receipt of your
Registration Form. Wire transfer process must be initiated within 2 weeks from
the date of your invoice or full payment must arrive at the QFD Institute by
October 3, 2008, whichever comes first. Please
instruct your purchasing department and bank to add any bank fees that are
associated with wire transfer services, on top of your total registration fees. In the
event of cancellation by the registrant, the general refund policy applies minus
applicable bank fees. Your registration will be confirmed upon receipt of full
payment.
(Note: Some countries may be beyond the coverage of our bank's wire transfer
service. If so, you will be notified by email within 24 hours and will be
asked to select an alternative payment method.)
Walk-in Registrations:
Walk-ins are welcome for the Symposium but reservations are
recommended to ensure your seat and materials. All QFD Belt Courses require
pre-registration due to limited class sizes. Full payment is due before the
start of all programs.
Late Registrations & Cut-off Date:
QFD course registrations are accepted on a
first-come-first-served basis, and may close subject to seat availability
or at 12:00 pm Noon one (1)
business day before the start of the course, whichever comes first. QFD Belt
course registrations received within 48 hours before the start of the course may
be subject to a 20% late surcharge to cover extra costs in material preparation.
All deadlines are by U.S. Eastern Time.
Cancellations after Registration & Substitutions:
Full refund or credit will be given when cancellation is notified
by a pre-paid attendee by 5:00 pm September 5, 2008. No refund will be made after that. However, substitutions are
permitted without penalty, provided they meet the prerequisites. All deadlines
are by U.S. Eastern Time.
Course Cancellations by the QFD Institute: In the event of course
cancellation by the QFD Institute, the QFDI's liability is specifically limited
to refunding the applicable course and/or program fees and excludes any
incidental or consequential damages.
Group/Government Discounts:
For Value Packages and QFD Belt® Courses, a 5% per
person discount may be applied to group registration of three (3) or more people
from the same company submitted at the same time, as well as for registrations of U.S.
federal government employees with a valid ID. For Symposium, $20 per person
discount may be applied to group registration of three (3) or more people from
the same company submitted at the same time, as well as for registrations of U.S.
federal government employees with a valid ID. Group Discounts, Government Discounts,
Speaker Discounts, and Student Discounts may
not be combined in any way.
Corporate or School-issued Email / Overseas P.O. Box Address:
All registrations must accompany your corporate
email address issued by your employer or a school-issued email address if you
are a student or faculty, due to new security requirements by credit card banks.
Registrations with a free domain email address will not be accepted.
Registrations using an overseas post box address will not be accepted.
QFD Institute reserves the right to refuse sales of any product
or service to anyone or any organization. Registrations with untrue, inaccurate,
or incomplete information may not be processed.
